Revised Applications Project Update (O.NB.F.B/PR.H)
IMPORTANT: When Virginia originally approved the new applications in March 2007, the package incorrectly included a nationwide version of the Notice to Proposed Insured form. Virginia has a state-specific Notice to Proposed Insured form (AGLC102112-VA).
Also, new application forms have been recently approved in Utah. Old forms will not be accepted after September 2, 2007 for both Utah and Virginia.

As the new forms are approved in remaining states, you will have a 60-day transition period. The new forms are currently available on FastForms, via our Web site, www.aiglifebrokerage.com.
