Accelerated Death Benefit Disclosure Requirement (O.COP.F/P.PR.H)
Effective immediately, an
Accelerated Death Benefit
(ADB) disclosure form must be completed at the time of application for any life insurance
product sold with an ADB rider. This includes Term, Whole, Universal Life and Variable
Universal Life cases. New forms will be required:
